Below we have outlined the key points of what we do, and our future goals for the company. We are passionate about providing the very best care and support in Cardiff.
Our vision is to provide person centred; outcomes focussed, support based on an attachment/trauma-based model of care. This will be planned to maintain and increase the service user's independence and active participation both in the community and in planning their own support.
Our service users and their families, will be able to rely on our service being professional, consistent, and supportive for as long as we are needed. Whilst we will put the vulnerable person at the heart of everything we do, we will also ensure that their families feel supported and confident in our work and see the value of what we do.
We will help the vulnerable person to engage with the local community and developing long term sustainable change. This is not an exhaustive list as flexibility is important in responding to the changing needs of our clientele. Flexibility will be a key feature of the service we offer, and as this is a new organisation, we will be looking to expand our workforce, and their skills, to meet the needs of the community.
We provide daily community support for vulnerable people with complex learning difficulties, ALN/SEMH and challenging behaviour to broaden their social experience, contribute towards their self-confidence, independence and social skills. We will also ensure that their families feel supported and confident in our work and see the value of what we do.
It is very important that the service user enjoys being with us and values our service offering meaningful intervention for themselves, giving them space, time, and someone to talk to away from the pressures of family life, allowing them to develop their wellbeing and life-skills.
We are able to offer an education programme for young people who are currently not in a formal education provision. With two ex-headteachers, both primary and secondary, we are able to provide a tailor-made curriculum suited for each individual.
It is imperative that we form a trusting relationship with the young person to ensure that they enjoy learning again.The long term goal would to then ease them back into formal education once they feel confident with being back in a learning environment.
We aim to hire venues to host planned activities such as game and activity evenings for our vulnerable people to socialise together.
The idea of this is so that our service users can form new friendships, boost their independence and have a safe environment which they can look forward to visiting each week.
Each activity will be planned in mind and tailored to the interest of those who wish to attend.
Azalea Care and Education Ltd (ACE) specialises in the care and support of individuals, with ALN/SEMH and challenging behaviour.
We are registered with CIW as a care organisation and our services are delivered in line with guidance from Welsh Government, regulatory bodies, and the local authority.
We deliver community and domiciliary based care and support to meet the physical and emotional needs of our service users. This includes modelling appropriate care and support within the family setting. We help the service user to engage with the local community and develop long term sustainable change.

DownloadThe starting point for a referral will come from the allocated social worker of the individual, and they will provide as much information surrounding the package of care required. Once this is received, a senior team member from ACE will then make a visit with the family, where possible, to gain further information.
Whilst we will welcome all referrals from the Local Authorities, ACE will collate all information from the referral form and initial family visit to assess whether we are able to provide appropriate support. If we are able to meet the needs of the individual's package of care, a tailor-made care plan will then be written up for the individual.
Care plans for each individual will then be closely monitored and periodically reviewed.
